I understand and agree to the Jr. High Youth Rally Code of Conduct and Dress Code (found to the left ). I understand that my parent/guardian will be notified at the time of any infractions requiring my dismissal from the Rally and must come to take me home immediately.  

By checking the box below I (the participant) agree to the Code of Conduct and Dress Code.

I, the parent/guardian of this child, grant permission for my child to participate in the Junior High Youth Rally to be held at Roncalli High School on February 1, 2020 

I will not hold Nativity Catholic Church, the Archdiocese of Indianapolis, Roncalli High School, chaperones or representatives associated with the Junior High Youth Rally responsible in the event of injury. Further, I agree to accept any and all financial responsibility as a result of scheduling necessary emergency medical treatment. I will not hold my parish responsible for any injury incurred on the trip to and from and during the Junior High Youth Rally. I hereby warrant to the best of my knowledge, my child is in good health and I assume all responsibility for the health of my child. In the event of an emergency, I hereby give permission to transport my child to a hospital for emergency medical treatment. I wish to be advised prior to any further treatment by the hospital or doctor.

This participant may be photographed and/or videotaped while participating in the Jr. High Youth Rally. Photos and videos could be used for future promotion of the Rally or other events.
